Job Description

As an OOHC Case Manager you will be responsible for the overall case management, case coordination and monitoring of placement quality for childrena nd young people in Out of Home Care in line with OOHC standards and legislation. 

  • Support & coordinate a child or young person’s needs through a multi-disciplinary lens, focusing on health, wellbeing, developmental, educational, therapeutic, and cultural needs.
  • Practice with an in-depth understanding of cultural awareness
  • Collaborate with all stakeholders in a child or young person’s network, including family, extended kin, service providers and carers.
  • Monitor and review placements for children and young people’s safety and wellbeing.
  • Report all incidents of abuse and neglect as per mandatory reporting responsibility to the Child Protection Helpline.
  • Develop case plans that prioritise permanency goals of children.
  • Plan and facilitate case plan meetings for children and young people.
  • Work collaboratively with CARTS Team and ensure appropriate placement matching, placement review and placement transition plans for children and young people, with carers who can meet their safety, wellbeing, therapeutic and permanency outcomes. 
  • Explore viability of restoration to birth families.
  • Facilitate restoration of children and or young people to their birth family as directed by the Department of Communities and Justice, following a children’s court order

  • Relevant degree-level qualification in social sciences or similar field OR Minimum proven 2 years’ experience in OOHC Case Management experience WITH a diploma-level qualification social sciences or similar field
  • Understanding of the Permanency Support Program (PSP) and its guiding principles including a commitment to family finding, restoration, and permanency.  
  • Demonstrated in depth knowledge of trauma informed practice and managing challenging behaviours. 
  • Demonstrated understanding/awareness of child protection legislation, identifying, and assessing risk of harm for children and young people.
  • Demonstrated commitment to empowering and advocating for children and young people to have a voice in decisions that affect them.
  • Experience working with children, young people, and families from CALD & Aboriginal & Torres Strait Islander backgrounds. 
  • Ability to prioritise and meet deadlines while working independently.

  • National Police History
  • Computer literacy including a sound level of competency with Microsoft Office platforms and client management systems. 
  • Current NSW drivers’ licence
