Job Description
As an OOHC Senior Case Manager you will be responsible for the overall case management, case coordination and monitoring of placement quality for children and young people in Out of Home Care. The role also requires some level of mentorship to new case managers.
Duties & Responsibilities
- Support & coordinate a child or young person’s needs through a multi-disciplinary lens, focusing on health, wellbeing, developmental, educational, therapeutic, and cultural needs.
- Collaborate with all stakeholders in a child or young person’s network, including family, extended kin, service providers and carers.
- Advocate and consider the voice of children and young people in service delivery and decision making.
- Follow the principles of trauma informed care and best practice for managing challenging behaviours.
- Monitor and review placements for children and young people’s safety and wellbeing.
- All other case management responsibilities.
